Projects for Assistance in Transition from Homelessness (PATH)

The Projects for Assistance in Transition from Homelessness (PATH) Program supports the delivery of outreach and services to individuals experiencing homelessness and diagnosed with a serious mental illness. Over 500 providers are involved with the PATH Program nationally. In North Dakota, the Behavioral Health Division within North Dakota Health and Human Services administers the PATH program with services provided by the regional human service centers.
